Notice to Our Patients
11/1/2013
Our physicians and staff strive to provide the best possible medical care in a personable atmosphere and we
will continue that philosophy. Many changes have come to pass this year in healthcare.
1. Exam Classification Policy
Due to the mandatory changes effective on October 1, 2013, our physicians and staff are making necessary
changes to remain compliant If the physician determines the patient is in the office for a non-medical reason
(without a medical problem, symptom or complaint), the care provided is non-medical and will be billed to the
patient and/or non-medical vision insurer. If the physician determines the patient is in the office for a medical
reason (with a presenting problem, symptom or complaint), the care provided is medical and will be to the
patient and/or medical insurance.
What does this mean for you, our patient? You may be asked to schedule another appointment or may be
responsible for additional co-payment on the same day.
2. Payment for Services
Our staff will do our best to determine eligibility for our patients. Most major insurance companies have a
website to view your benefits. You will be asked to pay the patient responsibility at the time of service
including (but not limited to) co-payment, deductible and contact lens fits. Please keep in mind insurance
coverage is a contract between you and the insurance company. Our staff will not guarantee payment from
your insurance company for any service.
What does this mean for you, our patient? We advise calling your insurance company prior to the visit to
obtain your policy benefits and financial responsibility.
